Add Metadata Base Forms

Metadata base forms capture information related to subject informed consent signing, over-volunteering, and study eligibility. These base forms must be added before a study can go live.

Base Forms are used to facilitate specialized workflows in ClinSpark. They can be added in CRF Design > Study Library module.
